Veterinary referral

Pet behaviour counselling can only be started after veterinary referral. It is against the law to carry out any counselling without this permission.

To give you the best service possible it is neccesary that I gain a full understanding of your pets history. To allow this to happen you must fill out the attached veterinary referral form. By completing this form your vet is aware of the situation and can release your pets records to me, so I can tailor your service specifically to you.

Mention to your vet that you would like an appointment with Pet Mentors and give this referral form to them so we can get your pets training off on the right foot.

The relationship I look to develop is one of a team between you, your vet and I. The relationship between your pet and you is paramount and between us we need to work together to make sure your pet is happy.
